Get Birth Certificates by Mail in 2007 

Starting Friday, December 15, the Office of Vital Records at 1090 Commerce Drive will adopt a new mail-only service for people seeking birth certificates. Parents (or legal guardians) who have children needing birth records for school or other purposes have some options. Application forms are available either in the Yavapai County Community Health Services (YCCHS) lobby, which is open Monday through Friday (except holidays), or online.  Click here to print one in English or Spanish.

Sign the application form in the presence of a notary public or make copies of a government-issued photo ID card. Add your personal check or money order and submit your application and supporting documents to YCCHS by either dropping it off in the lobby lock box, or mailing it to Yavapai County Community Health Services, 1090 Commerce Drive, Prescott, AZ  86305.
